I think that is most unlikely. Cock-up rather than conspiracy. But after the cock-up a great deal was wrong that should not have been.

I presume that is in response to me. I didn't mean that de Menezes wasn't a cock-up, rather I was suggesting that there was a policy decision, or at least policy-mooting, to kill if the opportunity arose. De Menezes just happened to step out into that environment.

Yes, that's what I think is rather unlikely. There are simpler explanations.
