Houses weren't built with outside loos. Houses were built without loos. The loos were added later as close to the main sewer as possible to keep costs down.
I've been watching the 'history of rooms' or whatever it is called on BBC4. Last night was about the kitchen. Really interesting how our social and housing habits evolved, and how cheap energy in the form of coal, gas and electricity have influenced house design and behaviour for three centuries.
Probably repeated later in the week and on iplayer.

Let nobody suppose that simple, inexpensive arrangements are faulty because primitive. If constructed correctly and in line with natural laws they are not only right, but preferable to fancy complicated devices.
